Police resources tell TMZ... the actor had been found Sunday morning in a very Greenwich apartment... and they are still on view investigating.
Other sources say... Hoffman was found in his bathroom within his boxer shorts, with a needle sticking out of his left hand. We're told law enforcement officials found 10 glassine envelopes -- 2 from the envelopes had heroin... others presumably were vacant.
One of the actual envelopes had "Ace of the Spades" written on it, and another had an ace involving hearts symbol -- such symbols and words in many cases are markers from people that make heroin -- to brand this.
Law enforcement sources say TMZ... Hoffman was supposed to sweep his kids with 9 AM Sunday but did not show -- which has been unusual for him. A playwright and a friend became anxious, came over along with found the actor alone inside apartment.
Details around his death tend to be unclear... but this actor had fought with drug complications... most recently checking himself in to a rehab facility for heroin back in the May.
TMZ broke the actual story of Hoffman about to rehab. We were told he did it as a prevention measure because he was approaching a major relapse. A source associated with Hoffman's treatment right now tells TMZ... the actor said back then he was around the brink of being a heroin IV user and needed to nip it inside bud before that happened.
As most of us previously reported... Hoffman was clean for 23-years but fell over wagon in 2012.
Apparently Hoffman's last interview was on the Sundance Film Event with XiXi Yang from PopStopTV for its movie "A Wanted Man. "
Hoffman's family members released a affirmation saying that, "We're devastated by losing our beloved Phil along with appreciate the outpouring involving love and assistance. "
Law enforcement sources reveal his autopsy will require place Monday.
